Out-Of-School Care Registration - for September 2012
The Fairfield Gonzales Community Association (FGCA) provides care to Margaret Jenkins Elementary, Sundance Elementary and Sir James Douglas Elementary. Registration takes place at 1330 Fairfield Rd. on a first come first served basis on the following dates:
Saturday February 11th, 2012, 9:00am‐4:000pm for *CURRENT OSC Families*
Saturday February 18th, 2012, 9:00am‐4:000pm for *NEW Familes*
OSC Registration forms for 2012 -2013 will be available Monday February 6th.
EXPECTATIONS FOR YOUR REGISTRATION PAPERWORK
All legal guardians/parents must read the new parent handbook
All legal guardians/parents must provide their signature on this internal document
If you are separated/divorced we require a current copy of all legal custody agreements referencing custody arrangements ONLY
If you are separated/divorced and do not have a legal custody agreement we require an FGCA custody arrangement document.
This document must be updated as applicable. (Please ask your on-site manager for direction prior to registration)
Provide a copy of your child’s immunization
If your child has a special need or behavioural need you must attach a care plan (Please ask your on-site manager for direction prior to registration)

CHEQUES AND FEES REGULAR MONTHLY FEES /SUBSIDY FAMILIES
New families are required to provide a one-time only administrative fee of $30 One cheque per family is acceptable
Provide a non-refundable $150 deposit dated for the registration date
Deposit is non-refundable and is placed against your last month of care (June)
Should you withdraw at an earlier time your deposit will be placed against the last month of care provided that you follow the FGCA withdrawal policy as per the parent handbook
All cheques are made out to: The Fairfield Gonzales Community Association
Memo Line: Your Child’s Name and either MJOSC/FFOSC/SDOSC
Provide 10 cheques dated for the first of each month commencing in September 2012 and completing in June 2013
o September through May are your child’s regular fees
June will be your regular fee less your $150 deposit Subsidy families are required to submit cheques for the full monthly, Pro D Day and camp fees. Upon the FGCA receiving child care subsidy monies, we will issue you a credit, refund or exchange as necessary. All un-used cheques will be returned or shredded unless your account is in arrears.

WINTER/SPRING BREAK CAMP
Should you require/request Winter Camp and/or Spring Camp Care, provide a cheque for the total amount of Winter Care you require as well as a cheque for the total amount of Spring Care you require. Camp is $30 per day and $150 for the week. o Winter Camp will be provided Monday December 31 2012 Wednesday January 2, 2013

Probationary Period
Each new child is accepted on a one month probationary period. Based on our discretion, the FCA reserves the right to refuse care. During the first two weeks of this probationary period, we will be looking at how your child fits into our program. If at this time, we feel that our program does not effectively meet your child’s needs, we will provide you with notice to make alternate care arrangements.
Withdrawal and Cause for Discharge
To withdraw or change registered days without penalty, families are required to give one month’s written notice, received by the 1st day of the preceding month. If one month’s notice is not given, you will be charged one month’s fees in lieu of notice.
Grounds for discharge or dismissal include, but are not limited to:
- Three late payments, NSF cheques or non-payments
- Three late pick-ups
- Inability of the child or family to follow FGCA expectations and policies
- Inability of the program to meet the needs of the child or family
